Why 8% of Health Insurance Claims Get Rejected — and How to Not Be One of Them

The Scale of the Problem

In FY 2024-25, Indian health insurers processed 3.26 crore claims worth ₹94,248 crore. Of these, 8% were rejected or partially disallowed — a slight improvement from 11% in FY 2023-24, but still representing lakhs of families who paid premiums and were denied at the time of need.

More telling: 69% of all grievances filed with IRDAI's Bima Bharosa portal were claim-related — delays, partial settlements, or outright rejections.

Top 4 Reasons Claims Get Rejected (IRDAI Data)

IRDAI's regulatory audit identifies four structural reasons behind most rejections:

1. Sum Insured Exhausted — Policyholders exceed their annual limit. A ₹5 lakh policy and a ₹6 lakh bill means a ₹1 lakh shortfall.

2. Hidden Sub-limits — Room rent capped at 1% of SI means in a ₹10L policy, you get ₹10,000/day. Private hospital rooms in metros routinely cost ₹15,000–₹25,000/day. The proportionate deduction rule kicks in — your entire bill gets reduced proportionally.

3. Pre-Existing Disease Non-Disclosure — Conditions not declared at the time of buying the policy. Insurers investigate claims above ₹1 lakh routinely.

4. Non-Payable Consumables Deducted — IRDAI Annexure IV lists items insurers can legally exclude: cotton, gauze, syringes, disposable gloves, thermometers, infusion pumps, toiletries. A hospital bill of ₹2 lakh can have ₹15,000–₹30,000 in non-payables.

What IRDAI Mandates (and Insurers Don't Advertise)

Under IRDAI Master Regulations 2024, cashless pre-authorisation must be approved within 1 hour of the request. Final discharge authorisation must be given within 3 hours. If your insurer takes longer, you have grounds to escalate immediately to the Bima Bharosa portal.

81.13% of all claims are paid within 3 months of intimation. 16.3% take 3–6 months. If your claim is pending beyond 6 months without explanation, file a grievance.

How to Protect Yourself

Before buying: Declare all pre-existing conditions in writing. Get acknowledgment. Choose plans with no room rent sub-limit if you plan to use private hospitals.

At the hospital: Ask for an itemized bill. Request the hospital to separate non-payable items before submission. Consumables add-on riders (available with Niva Bupa, Star Health, HDFC ERGO) eliminate most deductions.

If rejected: File on Bima Bharosa portal. 93% of grievances filed in FY 2024-25 were resolved within the year. If unresolved, approach the Insurance Ombudsman — it is free and binding on insurers up to ₹50 lakh.
