The Key Differences

Sum Insured: PM-JAY offers ₹5 lakh per family per year at zero cost. Commercial plans range from ₹3 lakh to ₹1 crore and above, priced by age and health.

Waiting Periods: PM-JAY has zero waiting period for pre-existing diseases — Day 1 coverage. Commercial plans impose 1–4 year PED waiting periods.

Co-payments: PM-JAY has absolute zero out-of-pocket for any listed package. No consumable deductions. Commercial plans routinely apply 10–20% co-pay for seniors, plus room rent capping and non-medical deductions.

Hospital Network: PM-JAY works only at empanelled hospitals. Commercial plans have wider access to private hospitals via TPA tie-ups.

Post-Hospitalisation: PM-JAY covers only 15 days post-discharge. Commercial plans cover 60–180 days.

Domiciliary Treatment: Not covered by PM-JAY. Most commercial plans cover it.

Maternity: PM-JAY covers from Day 1 under reproductive health packages. Commercial plans impose 2–4 year waiting periods.

Global Cover: PM-JAY has no international coverage. Premium commercial plans offer global critical illness cover.

When PM-JAY is Better

PM-JAY is genuinely superior in several ways that no commercial plan can match:

  • Zero premium — the ₹0 cost is the most important feature for BPL and lower-income families
  • No PED waiting period — immediate Day 1 coverage for existing conditions
  • No consumable deductions — hospital cannot bill the patient separately
  • Maternity from Day 1 — commercial plans make you wait 2–4 years

For the 55 crore people it covers, PM-JAY is a life-changing safety net.

When Commercial Insurance is Better

For middle-income earners who can afford ₹10,000–₹30,000 per year in premiums, commercial plans offer depth that PM-JAY cannot:

  • Sum insured up to ₹1 crore — critical for major surgeries, cancer, cardiac events
  • Private hospital access — room of choice, specialist of choice
  • Post-hospitalisation coverage up to 180 days — medicines, follow-ups, diagnostics
  • Domiciliary treatment — home care covered
  • NCB (No Claim Bonus) — sum insured grows each claim-free year
  • Restoration — SI automatically replenished once exhausted in the year

The honest answer: PM-JAY for safety net. Commercial for real financial protection.
